Supports more than one housing, homeless services, or community support program based on current agency needs, which can include: client intake, eligibility review, case management, goal planning, referrals, follow-up, documentation, and coordination with internal programs and outside community partners.
Position Summary
The Case Manager provides case management services to individuals and families who are experiencing housing instability, homelessness, or related barriers. This position supports housing stability through assessment, service planning, resource coordination, and regular client follow-up.
Reports To: Housing Director and Community Services/Shelter Director

- Documentation and program compliance
Core Duties
- Conduct intakes, assessments, and goal planning with clients
- Provide direct case management, follow-up, and support services
- Help clients access housing, benefits, employment, transportation, and other needed resources
- Coordinate services with ICCAP staff, landlords, community partners, and outside agencies
- Support clients with budgeting, life skills, crisis needs, and independent living goals
- Maintain accurate case files, service plans, notes, and required records
- Conduct home visits or field work as needed to support client stability
Minimum Qualifications
- Experience in social services, case management, housing, homeless services, or a related field preferred
- Strong communication and organizational skills
- Ability to work independently and manage multiple responsibilities
- Knowledge of community resources and human service systems preferred
- Basic computer and documentation skills
- Ability to work professionally with clients, partners, and team members
Requirements
- Valid Pennsylvania driver’s license, reliable transportation, and proof of vehicle insurance meeting agency standards
- Willingness to travel in Indiana County and other counties as required
- Ability to obtain required clearances, including Motor Vehicle Report, Act 33, Act 34, and FBI clearance with fingerprinting
- Ability to maintain confidentiality and handle sensitive client information appropriately
Work Environment & Expectations
This position includes both office and field-based responsibilities with regular client and partner interaction. The work is fast-paced and requires flexibility, follow-through, and strong documentation habits.
