Programming focused on helping individuals address barriers such as housing, food, transportation, health access, and other basic needs.

Position Summary

The Community Health Worker Liaison supports individuals and families by providing education, outreach, and connections to healthcare and community resources. This position helps clients identify needs, overcome barriers, and access services that support health, stability, and independent living.

Reports To: Direct Services/Shelter Director

 

Areas of Responsibility

  • Community outreach and engagement
  • Client support and case coordination
  • Health education and resource connection
  • Needs assessment and service planning
  • Documentation and reporting

Core Duties

  • Conduct outreach to engage community members and promote available services
  • Assess client needs and develop individualized support plans
  • Help clients navigate healthcare, human services, and community resources
  • Coordinate referrals and follow up to ensure needs are addressed
  • Build positive relationships with clients, service providers, and community partners
  • Maintain accurate, timely, and confidential documentation
  • Participate in trainings, meetings, and community education efforts

Minimum Qualifications

  • Experience in social services, community health, or a related role preferred
  • Strong communication, listening, and interpersonal skills
  • Project coordination and organizational skills
  • Ability to manage multiple tasks, meet deadlines, prioritize duties in a timely manner
  • Ability to work independently and as part of a team
  • Basic computer and data entry skills

Requirements

  • Bachelor’s degree in a related field preferred or equivalent experience
  • Valid driver’s license, reliable transportation, and willingness to travel as needed
  • Ability to obtain required clearances, including Act 33, Act 34, and FBI clearance with fingerprinting
  • Willingness to obtain Community Health Worker certification if required
  • Ability to maintain confidentiality and work respectfully with diverse clients and communities

Work Environment & Expectations

This position includes office, field, and community-based work with regular client and partner interaction. Some evening or weekend availability may be needed to support program goals.
